Attracting plastic surgery clients with Facebook Instagram ads

In this case study, we will show the results of the advertising campaigns of a plastic surgery clinic in Turkey.

Here we also used the method of bypassing moderation on Facebook and Instagram in order to create unique adverts and bring the client the maximum ROI on their advertising investment
About the project:

Plastic surgery clinic, is located in Turkey (currently we are cooperating with it under NDA agreement and unfortunately we cannot write the name of this clinic, by agreement we can only share the results of advertising campaigns).

Medical tourism is very well developed in Turkey, more than 70% of people from Europe and North America choose clinics in Turkey for plastic surgery. Here are pleasant prices for surgery, compared to Europe and America, and the quality is no worse.

Our clinic used to get new clients by advertising on Google, Facebook, Instagram and Pinterest. But they always had problems with the constant flow of applications and their quality. Most of the slippage was on Facebook and Instagram - expensive submissions, most of which were untargeted.

That's why they turned to our agency.


The goals of the advertising campaign:

  • Create a stream of targeted applications coming from unique adverts
  • Ensure a smooth flow of traffic and successfully spend $9,500 of the advertising budget

Stages of advertising campaign implementation:

Stage 1: Create and launch unique adverts to bypass Meta moderation

Since we have quite a lot of experience in promoting plastic clinics, we have formed our own bundles that give the best results in advertising.

We know that before/after adverts work well. This happens because we immediately show a potential client's pain in a "before" format from the first second of interaction with them. It could be small breasts or buttocks, being overweight, baldness, a hump on the nose, etc. But immediately we show the "after" format in which the person sees the desired result and this is what makes him read our advert to the end and leave a request for a consultation. In the text we just highlight the key advantages of this operation or procedure, more detailed information the client will find out in consultation with the manager.

Due to the fact that the Meta moderation considers that "before/after" adverts may discriminate against some people, this ad format is prohibited. But these bans do not affect us in any way. We have no problem running ad creatives of this format in adverts and getting the best results on the market, while competitors are trying to pass Meta moderation by making veiled adverts that don't hit the target audience's pains in any way. We have no problem sharing our working creatives in our case studies, because without our help you won't be able to pass moderation with such adverts.

Stage 2: Analysing and scaling advertising campaigns

After the first day of running the ads, we received over 60 bids at $4.5-5. Of course, some of the bids were invalid and there is no way to protect against that, but we are constantly working on reducing the number of bad bids and that's why at the beginning of the first few days of cooperation, every day we ask for feedback from the bid quality managers. They give us information about how many applications had wrong numbers, how many people cancelled due to price or other factors. We collect this information, analyse it and make adjustments to the advertisements so that the ads are shown only to our target audience.

After a few days of constant tweaking of the adverts, we reached the desired result in terms of the number of real applications and the number of new clients and after that we started to scale our advertising campaign, increasing the amount of advertising budget per day.

In the launch we use several advertising accounts so that if one of them is blocked, we can resume showing ads on the other one without losing traffic. We share the results of advertising campaigns on one of the advertising cabinets.
Results of the advertising campaign:

During the first month of cooperation we spent $9,500 advertising budget and received 1,895 applications for various plastic surgeries and procedures. The average price of a request was $5. Of course, the bid price depends on the type of operation, the country where the advert is shown, the price per operation, etc. In this case we consider all advertising campaigns together. But these figures mean nothing without the real profit from advertising. We know that advertising should bring money. Therefore, we always focus not on the number and price of applications, but on their payback.

At the moment 32 clients with our adverts have already had plastic surgery at the clinic. The only information we can announce is the profit from the operations, it is 128.000$, the average cheque of one operation - 4.000$.
At the same time, many clients are in the negotiation stage, and 17 have already made an advance payment for future surgery, this money we did not include in the net profit.

At this point, the payback from advertising is 92.5%. We believe this is a very good result and we are currently scaling our advertising campaigns to generate even more profit.

The results of advertising campaign ROI depend not only on the marketing agency, but also on the order processing department, the quality of services provided, etc. After all, if the managers of our clinic did not call back within 15 minutes after leaving an application by the client, we would not have such figures of payback. When every link in the chain is interested in the result, this often leads to very good results.


We are well aware of any aspects that can affect the outcome and give each of them the attention they deserve. We never focus on the number and price of applications, for us the most important thing is the return on investment in advertising.
